I acknowledge that participation at the 3PL Value Creation North America Summit includes possible exposure to and illness from infections diseases including, but not limited to, COVID-19. I voluntarily assume the risk of exposure or infection by attending the event. I further understand that the risk of becoming exposed to or infected by COVID at the event may result from the actions or omissions of other persons present, as well as their families, colleagues, or others with whom they may have contact.

I agree to comply with all COVID-related rules and procedures that may be implemented by Armstrong & Associates, Inc. or Embassy Suites event venue including, but not limited to, mask-wearing, social distancing, or other restrictions, in order to protect as much as possible the health and safety of all participants.

I hereby release, waive, and forever discharge any and all liability, claims, and demands of whatever kind or nature against Armstrong & Associates, Inc., Embassy Suites, and any affiliated partners, sponsors, contractors, or vendors including in each case, without limitation, their directors, officers, members, employees, volunteers, and agents (the "Released Parties"), either in law or in equity, to the fullest extent permissible by law, including but not limited to damages or losses caused by the negligence, fault, or conduct of any kind on the part of any Released Parties, including but not limited to death, bodily injury, illness, economic loss or out of pocket expenses, or loss or damage to property, which I, my heirs, assignees, next of kin, and legally appointed or designated representatives, may have or which may hereinafter accrue on my behalf, which arise or may hereafter arise from my participation at the 3PL Value Creation North America Summit, whether related to COVID or otherwise.
